A nursing home (known officially as a “care home with nursing”) provides the same support plus care from registered nurses, who are on duty around the clock. This article covers England; Scotland, Wales and Northern Ireland have their own rules, which we flag where they differ.</pee>

Key points

The core difference is nursing: a nursing home has registered nurses on site at all times, while a residential care home does not.

Residential care suits people who need everyday help but not regular nursing care. Nursing homes suit people with ongoing medical or nursing needs.

In England, your local council must assess your needs for free, whatever your savings. A separate financial assessment decides who pays.

Nursing homes may receive NHS-funded nursing care, and a small number of people qualify for fully funded NHS Continuing Healthcare.

Every care home in England is registered and inspected by the Care Quality Commission (CQC), so always read the latest report.

What is the difference between a residential care home and a nursing home?

<pee>Both are places where older people live, with meals, a room, company and support day and night. The difference lies in the type of care provided and who provides it.</pee>
<pee>A residential care home offers help with “personal care”. That means support with washing, dressing, using the toilet, eating, moving around and taking medication that has been prescribed. Care assistants (sometimes called support workers) deliver most of this care, under the direction of a registered manager.</pee>
<pee>A nursing home offers everything above, and also employs registered nurses. In England, the regulator describes these as “care homes with nursing”. A registered nurse must be on duty at all times, which allows the home to carry out clinical tasks such as managing complex wounds, giving injections, looking after catheters or feeding tubes, and monitoring unstable health conditions.</pee>
<pee>In everyday speech people often use “care home” for both types. When you are comparing options, check whether a home is registered for personal care only, or for nursing as well. Some homes are registered for both, and can support residents as their needs change.</pee>

Who is a residential care home for?

<pee>Residential care is usually right for someone who can no longer manage safely at home, but whose health is relatively stable. Typical reasons include:</pee>

needing daily help with washing, dressing and meals;

increasing frailty, with a risk of falls;

difficulty managing medication without support;

a carer who is no longer able to continue, or home care that has become unsafe or impractical.

<pee>Many residential homes also support people living with early or moderate dementia, and some have dedicated dementia units. A person with dementia does not automatically need nursing care. What matters is whether they have a nursing need as well as a need for personal care and supervision.</pee>

Who is a nursing home for?

<pee>A nursing home is right for someone whose needs require the skills of a registered nurse on a regular basis. This might include:</pee>

long-term conditions that need regular clinical monitoring or treatment;

complex or frequent wound care, or pressure sore management;

help with a catheter, a stoma, a feeding tube or injections;

recovery after a stroke or major illness, where nursing oversight remains necessary;

end-of-life care where nursing input is needed.

<pee>Whether someone has a “nursing need” is a professional judgement, not a matter of age or diagnosis alone. It is usually made by a social worker, a district nurse, a hospital team or a GP, sometimes through a formal NHS assessment.</pee>

How do the two types of home compare at a glance?

<pee>The table below summarises the main differences in England. Individual homes vary, so always check what a specific home is registered and equipped to do.</pee>

Feature

Residential care home

Nursing home (care home with nursing)

Main purpose

Accommodation and personal care

Accommodation, personal care and nursing care

Registered nurse on site

Not required; community nurses visit when needed

Yes, on duty at all times

Typical resident

Needs everyday help; health relatively stable

Has ongoing medical or nursing needs

Clinical tasks

Limited; nursing tasks are delivered by visiting NHS community nurses

Wound care, injections, catheters, feeding tubes and more

Funding

Council funding (if eligible) or self-funded

Same, plus possible NHS-funded nursing care or NHS Continuing Healthcare

Fees

Generally lower

Generally higher, reflecting the nursing staff

Regulator (England)

Care Quality Commission

Care Quality Commission

<pee>Tip: Ask any home you visit, “What happens if my relative’s needs increase?” A home that is registered for nursing, or that works closely with the local district nursing team, may avoid a second move later.</pee>

What does nursing care actually involve?

<pee>Nursing care is care that needs the skills of a registered nurse, or care that is planned, supervised and delegated by one. It is different from personal care, which a trained care assistant can deliver.</pee>
<pee>In a nursing home, the nurse on duty typically assesses residents’ health, writes and reviews care plans, administers medicines that require clinical judgement, carries out procedures such as dressing complex wounds, and liaises with GPs, hospitals, specialists and palliative care teams. Care assistants work alongside the nurses to deliver everyday support.</pee>
<pee>In a residential care home, nursing needs do not disappear. They are met by NHS community services instead. District nurses visit to change dressings or give injections, and GPs, physiotherapists and other professionals come in as needed. This works well for occasional needs. When nursing needs become frequent, complex or unpredictable, a move to a nursing home is often discussed.</pee>

Who pays for residential and nursing home care?

<pee>Care home costs in England are shared between individuals, councils and the NHS, depending on needs and finances. We do not quote fee levels or savings limits here because they change; the current figures are on GOV.UK and with your local council.</pee>
<pee>Council funding. After a needs assessment, the council carries out a financial assessment, looking at income, savings and, in some cases, property. People with substantial resources above the national upper limit pay for their own care (“self-funders”). People below it may receive help from the council, and may be asked to contribute from their income. Property is treated differently in some situations, for example when a spouse or partner still lives in the home.</pee>
<pee>Top-up fees. If a family chooses a home that costs more than the council would normally pay, a third party, often a relative, can sometimes agree to pay the difference. This is a long-term commitment, so read the agreement carefully.</pee>
<pee>NHS-funded nursing care. If a person lives in a nursing home and has been assessed as needing care from a registered nurse, the NHS pays a flat-rate contribution directly to the home towards the nursing element of the fees. This applies whether the person is self-funding or council-supported. It does not cover the whole bill.</pee>
<pee>NHS Continuing Healthcare. Some people have a “primary health need” and qualify for NHS Continuing Healthcare, where the NHS pays the full cost of care, including accommodation in a care home. Eligibility depends on a formal assessment of the person’s needs, not on a diagnosis or the type of home. It is not widely awarded, but it is always worth asking whether an assessment is appropriate, particularly when someone is leaving hospital.</pee>
<pee>Benefits. Some benefits, such as Attendance Allowance, may be affected when the council pays for a care home place. Check the current rules on GOV.UK or with an independent adviser before assuming either way.</pee>

How does admission work?

<pee>The process is broadly the same for both types of home, with some extra steps if nursing is involved. In England it usually runs like this.</pee>

Ask for a needs assessment. Contact the adult social care team at your council. Under the Care Act 2014, you are entitled to an assessment whatever your finances. A hospital discharge team can arrange it if you are in hospital.

Agree what kind of care is needed. The assessment should say whether you need personal care, nursing care or both, and whether other options, such as extra care housing or care at home, might work.

Consider NHS assessments. If there are significant health needs, ask whether an NHS Continuing Healthcare checklist or assessment is appropriate.

Complete the financial assessment. The council works out what, if anything, it will contribute and what you will pay.

Choose a home. You generally have a right to express a preference for a home, provided it is suitable, available and meets the council’s cost requirements. Self-funders can choose freely.

Visit and review. Visit more than once, at different times of day if possible, and read the CQC report.

Sign a contract and plan the move. Check what the fees include, notice periods and how fee increases are handled. Many homes agree a trial period.

<pee>If the person lacks the mental capacity to make this decision, the Mental Capacity Act 2005 applies. Decisions must be made in their best interests, involving the person as far as possible and consulting family and those close to them. In some cases, safeguards must be put in place if someone cannot leave the home freely, so ask the home and the council how this will be handled.</pee>

How do you choose a good home?

<pee>Whatever the type, quality matters more than the label. These steps help.</pee>

Read the CQC report. It rates homes on whether they are safe, effective, caring, responsive and well-led. Look at the date and the details, not only the headline rating.

Check the registration. Confirm whether the home is registered for nursing, dementia care or both, and whether it can meet the needs on your assessment.

Ask about staffing. How many staff are on duty by day and by night? How often are agency staff used? How long have key staff worked there?

Ask about daily life. What activities are offered? Can residents keep their own routines, bring furniture and go outdoors? How are families involved?

<pee>Advice for families: If you can, take your relative with you, or at least involve them in the decision. Ask about their routines, food preferences and hobbies, and check that the home can accommodate them.</pee>

How do the rules differ in Scotland, Wales and Northern Ireland?

<pee>Social care is a devolved matter, so the picture above applies to England. In the other nations, the basic distinction between care with and without nursing remains, but the regulators, funding and terminology vary.</pee>

Scotland: Care homes are regulated by the Care Inspectorate. Scotland has a policy of free personal care and free nursing care for people assessed as needing them, which is paid as a contribution towards care home fees. Accommodation costs (“hotel costs”) are still assessed financially.

Wales: Care homes are regulated by Care Inspectorate Wales. Local councils assess needs under the Social Services and Well-being (Wales) Act 2014, and funding for nursing care is organised through the local health board. Financial assessment rules differ from England.

Northern Ireland: Homes are regulated by the Regulation and Quality Improvement Authority (RQIA). Assessments and placements are arranged through the Health and Social Care Trusts, and nursing care costs are met differently from England.

<pee>If you live in one of these nations, use your own national government and health service websites for the current rules, and ask your local social work or social care team to explain the local process.</pee>

Can games and activities help someone living in a care home?

Activities and games to support memory and attention

<pee>Moving into a care home can be a big change, and meaningful activity is an important part of wellbeing in both residential and nursing settings. Families and care teams can use simple, enjoyable activities to keep the mind engaged. A few concrete examples:</pee>

Memory games on a tablet. Short, friendly exercises on recall, matching and word-finding that can be played alone or with a relative during a visit. The SCARLETT brain games for seniors are designed with older people in mind.

Reminiscence sessions. Looking at photographs, music and objects from the past, then talking about them, often prompts conversation and connection.

Small group activities. Quizzes, sorting games and word games that care staff can run, encouraging residents to take part at their own pace.

<pee>If you are supporting someone with memory problems, this course on understanding Alzheimer’s and everyday solutions is written for families. If needs increase, the home, the GP or the council can arrange a reassessment. Sometimes a person can stay in the same home if it is registered for both types of care. If a move is needed, families should be involved early, and the council or home should explain the reasons and options clearly.</pee>

Does dementia mean someone needs a nursing home?

<pee>Not necessarily. Many people with dementia live well in residential care homes, particularly those with experience of dementia care. A nursing home becomes relevant when there are also nursing needs, such as complex health conditions. The assessment should consider the whole person, not only the diagnosis.</pee>

Is the NHS contribution enough to cover nursing home fees?

<pee>No. NHS-funded nursing care is a fixed contribution towards the nursing element only. The rest of the fees are met by the person, the council or both. Full cover comes only through NHS Continuing Healthcare, which depends on a formal assessment of a primary health need.</pee>

Do we have to sell the family home to pay for care?

<pee>Not always. Property may be disregarded in some situations, for example if a spouse or partner still lives there, and a deferred payment agreement can sometimes delay a sale. Rules are detailed and change over time, so check GOV.UK and consider independent financial advice before making decisions.</pee>

Can we ask for an assessment even if we plan to pay privately?

<pee>Yes. In England, the council must carry out a needs assessment on request, whatever your savings. It can help you understand what type of care is appropriate, even if you later arrange and pay for a home yourself.</pee>

Sources

GOV.UK: guidance on care homes, paying for care and NHS Continuing Healthcare

NHS: “Care homes and nursing homes” and “NHS Continuing Healthcare” pages

Care Quality Commission (CQC): guidance on choosing a care home in England

Care Act 2014 and Mental Capacity Act 2005

Your local council’s adult social care team, for local assessment and funding rules

Care Inspectorate (Scotland), Care Inspectorate Wales and RQIA (Northern Ireland) for the devolved nations

<pee>This is an informative article.
