In England, national NICE guidance recommends that it is offered, and the research suggests modest but real benefits for thinking skills and quality of life, though it does not stop the condition progressing. This article covers England and notes where Scotland, Wales and Northern Ireland differ.</pee>

Key points

CST is a group programme of themed sessions, usually led by trained staff in community or care settings, for people with mild to moderate dementia.

It is one of the few non-drug approaches that national guidance in England recommends offering, and one of the best studied.

Evidence points to modest benefits for thinking skills and wellbeing; it does not cure dementia or halt its progression.

Availability varies a great deal from area to area, and the nations of the UK have their own arrangements.

Everyday games, conversation and reminiscence at home can complement a group, but they are not the same thing as CST.

What is cognitive stimulation therapy?

<pee>Cognitive stimulation therapy, usually shortened to CST, is a programme of group activities designed to stimulate thinking, memory and communication in people living with dementia. It is not a test, and it is not a lesson. The sessions are meant to be enjoyable, informal and respectful, and they aim to engage people in a way that feels natural rather than clinical.</pee>
<pee>The approach was developed by researchers and clinicians in the UK, notably at University College London and in partnership with other universities, and it has since been adopted in many countries. Its roots lie in earlier work on “reality orientation”, which could feel rather corrective. CST moved away from that. Instead of reminding people of the date and correcting them, it invites opinions, encourages new ideas and builds on what people can still do.</pee>
<pee>The underlying idea is simple. Mental stimulation in a supportive social setting may help people use their existing abilities more fully and feel more connected. That is a hope supported by research, not a guarantee, and it is worth keeping the claims modest.</pee>

How is a CST programme delivered?

<pee>The standard programme is run for small groups, typically a handful of people with mild to moderate dementia, led by one or two trained facilitators. It usually runs twice a week for several weeks, often around 14 sessions in total, each lasting about 45 minutes. Local services sometimes adapt the format, so it is worth asking exactly what is on offer where you live.</pee>
<pee>Each session follows a similar pattern so that it becomes familiar and reassuring:</pee>

A warm welcome. The group may use a name badge, a song or a short chat to settle in.

Orientation in a gentle form. A reminder of the day, the weather or the season, offered without pressure.

A themed activity. Typical themes include childhood, food, current affairs, famous faces, using money, word association, being creative, and physical games.

A closing moment. The group reflects on the session and says goodbye, often with a familiar song.

<pee>Facilitators are encouraged to value opinions rather than facts, to stimulate new ideas and associations instead of testing memory, and to adapt the activity to each person’s abilities. A person who cannot recall a name is not made to feel they have failed. This person-centred stance is arguably as important as the activities themselves.</pee>

<pee>Tip for families: ask whether the group follows the published CST manual and whether facilitators have had CST training. This is a reasonable question, and a good service will be happy to answer it.</pee>

Who is CST for?

<pee>Group CST is aimed at people with mild to moderate dementia. This includes Alzheimer’s disease and vascular dementia, among other types. Other things also matter for suitability: being able to tolerate a small group, having reasonable hearing and vision (or the right aids), and being able to take part in conversation in some way.</pee>
<pee>It is generally less suited to people with severe dementia, those who are very distressed or agitated in groups, or people with a serious untreated illness that makes concentration difficult. In those situations, other approaches such as one-to-one engagement, music, sensory activities or life-story work may be a better fit. The team that assesses someone for a group will take these things into account.</pee>
<pee>Many people living with dementia are also disabled people in the sense used by the Equality Act 2010, so services are expected to make reasonable adjustments. If a person has a hearing loss, a learning disability or a first language other than English, ask the service how they will adapt the sessions.</pee>

What does the evidence honestly say about CST?

<pee>CST is among the most researched non-drug therapies for dementia, and the picture is encouraging but measured. Here is what can be said with reasonable confidence.</pee>

Thinking skills. Reviews of trials, including a Cochrane review, have found small improvements in cognitive test scores for people who took part, compared with those who received usual care.

Quality of life and wellbeing. Several studies have reported better self-reported quality of life, communication or mood, though results are less consistent than for cognition.

No cure. CST has not been shown to change the underlying course of dementia. It is not a replacement for medical care.

Staying power. Benefits may fade after the programme ends. This is partly why “maintenance” groups were developed, but the evidence for continued groups is more mixed than for the original short course.

Quality of the research. Many trials are small and it is hard to blind people to what they are receiving, so some caution is appropriate about how large the true effect is.

<pee>In short, CST is a worthwhile option with a sound evidence base, and one that people often enjoy. It is not a miracle. Anyone selling it as one is overselling it. For the exact current wording of the evidence and the recommendations, read the sources listed at the end of this article.</pee>

Does NICE recommend CST in England?

<pee>Yes. The NICE guideline on dementia (NG97, first published in 2018) recommends that people living with mild to moderate dementia are offered a group cognitive stimulation programme, and that this is available regardless of any medication they take. NICE guidance formally applies in England; Wales usually takes account of it too, while Scotland and Northern Ireland have their own bodies and guidance.</pee>
<pee>It is worth being clear about what a recommendation means. It signals that, in the view of the guideline committee, the evidence supports offering CST as part of a care package. It does not mean that every area has enough places, nor that you can demand a specific group on a specific date. Capacity depends on local commissioning, which is why waiting times and formats differ so much.</pee>

Where can you find CST in England?

<pee>CST is usually delivered by a mixture of NHS and voluntary-sector providers. Common routes include:</pee>

Memory services and community mental health teams, which often run or signpost groups soon after diagnosis.

Your GP, who can refer you or put you in touch with local services.

Dementia charities, such as Alzheimer’s Society and local Age UK branches, which run groups or know who does.

Day centres and community venues, including libraries, church halls and community hubs.

Care homes, where staff may be trained to run CST for residents. The Care Quality Commission (CQC) inspects care homes in England, and its reports can show how well a home supports meaningful activity.

<pee>Services are not identical everywhere. Some areas offer CST soon after diagnosis; in others, there is a wait or no local group at all. If the first answer is “we don’t run it”, ask whether the voluntary sector does, or whether staff can suggest an alternative such as a singing group, a Dementia Café or a reminiscence group.</pee>

How does CST differ from other approaches?

<pee>People are sometimes confused by the range of dementia activities on offer. This table gives a simple comparison. It is a guide to how the approaches are generally described, not a ranking.</pee>

Approach

Format

Main aim

Group CST

Small group, structured themed sessions, trained facilitators

General stimulation of thinking, communication and enjoyment

Reminiscence work

Group or one-to-one, using photos, music and objects

Identity, connection and wellbeing through memories

Cognitive rehabilitation

Individual, goal-focused, with a therapist

Managing specific everyday difficulties

Music or creative groups

Group, informal, led by musicians or arts practitioners

Mood, expression and social connection

Everyday games at home

Informal, with family or carers

Enjoyment, shared time and gentle mental activity

<pee>These approaches are not mutually exclusive. Many people take part in a CST group and also enjoy music, walking groups or time with family. The best mix depends on the person’s interests and abilities.</pee>

Is CST available in Scotland, Wales and Northern Ireland?

<pee>Yes, though the routes and the wording of official guidance differ. Health and social care are devolved, so each nation has its own services and standards. In Scotland, dementia support is shaped by national standards and by local health and social care partnerships, and Alzheimer Scotland is a major provider. In Wales, health boards and local authorities commission services, and the national dementia strategy sets the direction. In Northern Ireland, support is organised through the health and social care trusts.</pee>
<pee>If you live outside England, the practical advice is the same: begin with your GP or memory service, then ask a national dementia charity about what exists locally. Avoid assuming that England’s guidance, referral routes or waiting times apply to you.</pee>

Can families support cognitive stimulation at home?

<pee>Families can, and often do, without realising it. A chat about the old days, a quiz, a sing-song, cooking together or a game of cards all involve many of the same ingredients as CST: conversation, choice, laughter and a sense of being valued. Studies of structured one-to-one CST at home exist, but their results are less clear than those for group CST, so it is best to regard home activities as a complement rather than an equivalent.</pee>
<pee>A few principles help:</pee>

Choose enjoyment over accuracy. If a quiz turns into a test, stop and do something else.

Follow the person’s interests. Gardening, football, hymns and old films all work as themes.

Keep it short and regular. A few relaxed minutes at a good time of day beat a long, tiring session.

Look after yourself. Carers are entitled to a carer’s assessment from the local authority, and support groups can make a real difference.

<pee>Good to know: if a game causes frustration, it is the activity that needs adapting, not the person. Simplify it, change it or leave it for another day.</pee>

Activities and games to support memory and attention

<pee>DYNSEO designs digital tools and training for people who support older adults. To be clear, these are not CST and are not a clinical treatment for dementia. They can, however, offer enjoyable activity between groups or alongside other support, and many families and care staff find the shared play useful.</pee>

SCARLETT offers memory and brain games for seniors, with activities that can be played together as a prompt for conversation and reminiscence. Brain-training apps are individual activities. They can be a pleasant addition, but they are not a substitute for CST.

Can someone with moderate or severe dementia take part?

National guidance in England recommends group CST for people with mild to moderate dementia. As dementia progresses, other approaches such as music, sensory activities and one-to-one engagement may suit the person better. The team running the group will assess whether it is right for them.

Do I need a diagnosis to be referred?

In most services, yes. CST is offered to people with a diagnosis of dementia, usually through a memory service, GP or community mental health team. If you are worried about memory, the first step is to speak to your GP.

Does CST slow the progression of dementia?

There is no good evidence that CST changes the underlying course of the condition. Research points to modest benefits for thinking skills and quality of life while the programme is running, and these benefits may not last once sessions stop.

Is CST available outside England?

Yes, in different forms. Scotland, Wales and Northern Ireland each have their own health services, guidance and local arrangements, so availability and referral routes differ. Ask your memory service, GP or a national dementia charity about local provision.

<pee>Sources and further reading:</pee>

NICE, Dementia: assessment, management and support for people living with dementia and their carers (NG97), 2018, as updated.
