If this is your first evaluation after the incident, start with NEURO1. The following forms will be proposed automatically based on your results.

NEURO1 Form

To be completed in the minutes following the head hit

A single "YES" to questions 1-20 indicates a high probability of concussion. Questions with ⚠️ are emergency signs.

NEURO2 Form

To be completed 1 to 3 hours after the fall/incident

This form includes a symptom questionnaire and cognitive tests to assess your condition.

TEST 1 Immediate Memory

Memorize these 5 words, then try to repeat them in any order. You will have 3 attempts.

Repeat these numbers backwards. For example, if the prompt is "7-2", you should answer "2-7".

TEST 3 Reaction Time

Ask someone to drop a 30cm ruler. Catch it as quickly as possible. Record the distance in cm where you caught it (average of 3 trials).

NEURO3 Form

This form prepares your return to sports activity. If all symptoms have disappeared, you can start the recovery program.

Recommendations to follow after a concussion

🚨 Do not stay alone

The concussed athlete should not stay alone during the evening and night following the concussion.

⚠️ Signs of deterioration - Call 15 or 112

  • Significant or increasing neck pain
  • Double vision
  • Weakness or tingling in limbs
  • Significant or increasing headache
  • Repeated vomiting
  • Seizures
  • Abnormal drowsiness
  • Increasing aggression or nervousness

🚫 To avoid

  • Alcohol and drugs for a minimum of 72 hours
  • Driving vehicles for a minimum of 24 hours
  • Intense physical activities for 48 hours
  • Prolonged screen exposure
  • Sleeping pills or strong painkillers

✅ Recommended

  • Cerebral rest: limit intense cognitive activities
  • Light activity: walking, strolling starting 24h
  • Nutrition: balanced, rich in omega-3
  • Sleep: regular and reasonable hours
  • Paracetamol: if headaches (short duration)

Gradual Return Program

Return to sports activity in stages

Each stage lasts at least 24 hours. If symptoms reappear, return to the previous stage.

Stage 1 - Relative Rest

Light daily activities, no sports. Limit screens and intense cognitive activities.

Stage 2 - Light Aerobic Activity

Walking, stationary cycling (10-15 min). Intensity < 70% max HR. No resistance.

Stage 3 - Sport-specific Exercises

Non-contact technical exercises. Running, specific movements.

Stage 4 - Non-contact Training

More intense training, coordination, and thinking exercises.

Stage 5 - Contact Training

After medical clearance. Gradual contact resumption.

Stage 6 - Return to Competition

Normal participation in games and competitions.

⚠️ Minimum delay

For cycling and sports with risk of falling: minimum 3 weeks (2 weekends without competition) before full return.
